Recommended Assessment Tools for the General Competencies
The Residency Review Committee agreed that the initial measurement tools developed and approved by the RRC to assess the ACGME Competencies should be revised. The first revision of these tools had been distributed to program directors approximately a year ago, and it was agreed that program director input would contribute significantly to their improvement and continued useability.
A subcommittee of Radiation Oncology program directors came to Chicago for a one-day workshop to discuss the documents and to accomplish the revisions. Deirdre Lynch, RhD, ACGME Research & Evaluation Specialist provided expert counsel to the group, and finalized the attached documents.

INSTRUCTIONS: Recommended Assessment Tools
These assessment tools are intended to be ‘suggestions’; their use is not required by the Residency Committee. Program directors should feel free to use them or not, and should modify them as appropriate to fit the needs of a program. The procedure evaluation forms need not be completed for all patients, but only for a representative sample.
All Documents are in PDF Format 
- Brachytherapy Form
This form may be used by faculty to evaluate a resident’s brachytherapy expertise. A representative sample of each major type of brachytherapy procedure should be evaluated periodically throughout a resident’s training.
- Simulation Form
This form may be used by faculty to evaluate a resident’s simulation expertise. A representative sample of cases of each disease type and stage may be evaluated periodically throughout the resident’s training.
- Communication, Interpersonal Skills, & Professionalism Evaluation Form
- Evaluation – Modified 360 Form
These forms were designed to be used periodically (once or twice a year) to evaluate a resident’s performance. Paramedical staff including radiation therapists, dosimetrists, secretaries or other relevant non-physician staff are appropriate evaluators.
- Oral Examination Form
If the program utilizes periodic mock oral examinations, then this form or a modification of it may be used to evaluate the resident’s performance on the specified examination.
- Global Rating Form
This form or a modification of it may be used by the faculty to evaluate residents after each major rotation.
