ACGME Accreditation Council for Graduate Medical Education

Evaluation Survey of Your Recent
Accreditation Site Visit

The ACGME hopes that all accreditation site visits are collegial and educational experiences for programs and sponsoring institutions, and that the experience is helpful to program directors or designated institutional officials.

Evaluation of the performance of the site visitor is important to improving the quality of the accreditation process. After a program has had its accreditation site visit, the Department of Field Activities sends an electronic letter and attached web-based evaluation form to the program director, asking for an evaluation of the site visitor (a member of the ACGME field staff).

Completion of the survey is an important way in which you can contribute to improving the site visit process. The sole purpose of the brief evaluation survey is to seek “actionable” feedback on the performance of the site visitor, to be shared with the site visitor in aggregated form, keeping the name of the program and evaluator confidential. Your responses will not be shared with the Residency Review Committee (RRC) or used as part of the evaluation of your program by the RRC. The site visit reports are also being evaluated by their RRC/IRC reviewer(s).

The electronic letter requesting your evaluation of the site visit should be sent within 7 to 10 days of the date of the visit. If you prefer to provide feedback verbally, please contact Ingrid Philibert at 312/755-5003 or Jane Shapiro, Associate Director, Field Activities, at 312/755-5015.

We greatly appreciate your assistance in providing this evaluation.
